1. Scope

This Privacy Policy describes how Ordonez Counseling, a Licensed Clinical Social Worker Corporation ("the Practice," "we," "us") collects, uses, and shares personal information when you visit ordonezcounseling.com, submit a form, or otherwise contact us (the "Site").

Health information that the Practice creates or receives in the course of providing clinical services , including evaluation records and reports, is protected health information governed by our Notice of Privacy Practices, which controls for that information. This Policy covers everything else.

2. Information we collect

Request and contact forms: name, email address, phone number, preferred language, whether you are an attorney or referring party, the type of proceeding, deadline, and any message you include.
Clinician applications: name, contact details, license type, license number and state(s), résumé or CV, and the information you provide in your application.
Email and phone communications: what you send us, and our replies.
Usage and device information: IP address, browser and device type, pages visited, referring page, and similar log data collected automatically by our hosting provider and any analytics tool we use.
Payment information: if you pay an invoice through a payment link, your card details are collected and processed by our payment processor. We do not store full card numbers.

3. How we use information

To respond to your request, prepare a quote, schedule an evaluation, or process a referral.
To review and process clinician applications and credentialing.
To communicate with you about the Practice's services.
To operate, secure, and improve the Site.
To comply with law, licensing requirements, and our professional obligations, and to enforce our Terms.

4. Sales and advertising

We do not sell personal information. We do not use information you submit through our forms, including any information about a legal matter or health history, for advertising, and we do not share it with advertisers.

5. When we share information

With clinicians in our network who will provide or support your services, under confidentiality obligations.
With service providers that act on our behalf under contractual confidentiality obligations: website hosting, email delivery, form processing, document storage, and payment processing.
With the attorney or referring party you identify, to the extent needed to coordinate your evaluation, and thereafter only as authorized in the engagement documents you sign.
For legal reasons: to comply with law, subpoenas, court orders, or licensing-board requirements, or to protect the safety of any person.
With your direction or consent.

6. Cookies and similar technologies

The Site uses only cookies that are necessary for it to function and to remember your language preference. We do not use analytics or advertising cookies. You can block or delete cookies in your browser settings; the Site will still work, though some preferences may not be remembered.

7. Security and retention

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information, including encrypted connections (HTTPS) and access controls. No system is perfectly secure. We keep form submissions and correspondence for as long as needed to respond to you and to meet legal and professional record-keeping requirements. Clinical and evaluation records are retained under California and federal record-retention rules, as described in the Notice of Privacy Practices.

8. Your rights and choices

You may ask us to provide a copy of the personal information we hold about you, correct it, or delete it (subject to legal retention requirements). Email info@ordonezcounseling.com with your name and the email address you used with us. We will verify your identity before acting and respond within the time required by law (and in any event within 45 days, extendable where the law permits). We will not discriminate against you for exercising your rights. You may use an authorized agent where the law permits; we will require proof of authorization.

Rights over your health records (access, amendment, restrictions, and accounting of disclosures) are described in the Notice of Privacy Practices and are exercised with the Practice under that notice.

California residents. The California Consumer Privacy Act provides California residents the rights described above. In the preceding 12 months we have collected the categories of personal information described in Section C.2, from you directly and from your device, for the purposes in Section C.3, and disclosed them for business purposes to the categories of recipients in Section C.5. We do not sell personal information and do not share it for cross-context behavioral advertising, and we have no actual knowledge of selling or sharing the personal information of consumers under 16.

9. Children

The Site is intended for adults. We do not knowingly collect information from anyone under 18 through the Site. Evaluations involving minors are arranged by a parent, legal guardian, or attorney, and the minor's information is handled as clinical information under the Notice of Privacy Practices.

10. Changes

We will post any changes here with a new effective date.
